Areas of Practice


Assessments

Most times, care management services begin with an assessment meeting with the client, caregivers and family to identify what is most needed and understand the client’s and family’s goals for the future. This meeting usually takes place at the client’s home—be it a family home or senior living to see how the client is managing their environment.


ongoing monitoring

When a family member or loved one lives out of town, or they don’t have the time, ability or knowledge to help out the way they would like, professional care management can step in. We provide ongoing observation and assistance to make sure our clients have the very best care and resources.


family conflict

Sometimes family members disagree on the best way to care for and support their elderly loved one. We can serve as a go-between in this situation to perform an assessment and make professional recommendations to help the family understand the situation more clearly, make informed decisions, and move forward in a positive direction.



care planning

Following an assessment, the care manager will often write up a review of the client’s current situation and make recommendations based on the needs assessment. This is called a “care plan”. Sometimes the assessment and care plan are all the client and family need to move forward and make recommended adjustments.


advocacy

Asking important questions of medical professionals, investigating an ambulance bill, challenging a Medicare denial—these are all common advocacy actions we take on a daily basis. We advise and advocate to get the best services and care that our clients are entitled to while supporting their wishes and desires.


navigating benefits/legal issues

Navigating the world of health insurance and social services—whether private, county, state, or federal—is a complicated necessity. We have resources to assist with finding the answers you need and completing applications. We can also refer you to skilled attorneys who specialize in elder law or wills/trusts/etc, if such services are required.



Information and referral

Giving information and making referrals to agencies/ professionals is a large part of our practice. Often, clients just don’t know where to turn for help. Whether you or your loved one needs meal delivery services, memory care, physical therapy, grief counseling, or help caring for beloved animals, we have the resources and network to support you.


quality of life

It’s not enough to survive, we all deserve to thrive! Care managers can serve as a link to a more enjoyable life. Examples include arranging social interactions, helping to plan a family visit and setting up custom activity plans. Providing seniors with opportunities to flourish and enjoy life is one of our main goals.


memory and end-of-life support

Memory and/or hospice care is big business these days, and it’s hard to know where to turn. Nobody should make these emotional and difficult decisions without awareness and support. We have years of experience with the differing levels of care and the variables involved in each. We can provide guidance to help you make the best choices possible.
